Payment of the $325 tuition constitutes agreement with terms and conditions stated in this form.  <br />
Ames Music Studios reserves the right to deny participation at our discretion.&nbsp;  Ames Music Studios reserves the right to terminate participation of any student at our discretion.&nbsp;  Sexually explicit or crude references in dress, speech, or behavior (including song lyrics) are not acceptable in practices or performance.&nbsp;  Parents, legal guardians of students, and students waive the right to any legal action for any injury sustained on property used for practices or performances resulting from any activity conducted by students before, during, or after practice or performance sessions.  Registration is secured only upon receipt of payment of $325 tuition ($165 becomes non-refundable deposit.)
